When performed by a qualified practitioner, osteopathic treatment and spinal manipulation is an effective and safe method of treatment for many painful conditions. There are however risks associated with any medical treatment, including osteopathic treatment, and I am required to inform you of these risks. You will have the opportunity to discuss the nature and purpose of osteopathic treatment at the time of your consult.

Please read the following carefully, and note any questions you may have. You will have the opportunity to discuss these with your practitioner at the time of your consult.

I understand, and am informed that, as in the practice of all medicine, in the practice of osteopathic medicine, there are risks to treatment including, but not limited to, muscle and joint soreness, muscle strain, joint sprain, fracture, disc injury, nerve injury, stroke and stroke-like episodes.

I do not expect the practitioner to be able to anticipate and explain all risks and complications. However, I wish to rely upon the practitioner to exercise clinical judgement during the course of my treatment, which the practitioner believes at the time, based upon the facts then known, is in my best interest.

I understand that results are not guaranteed.

I have read the above, and will ask my practitioner any questions I have about its contents at the time of my consult.

I hereby request and consent to the performance of osteopathic treatment to me by my practitioner.

I intend for this consent form to cover the entire course of treatment for current and future conditions for which I seek treatment.

I understand that I can withdraw consent at any time.
